Appointment Forms for New Hires - Law Clerks

Below you will find links to various appointment forms for your completion, as well as information regarding background checks and fingerprinting.  Please review the information carefully.  Unless the form is designated as “optional” all forms are required for your appointment with the Court. Detailed instructions are included for completion of each form. Please complete the forms on your computer and email to Beth_Mason@ded.uscourts.gov. Please  list your FULL LEGAL NAME on all appointment forms. Nicknames and preferences are not accepted. NOTE: Electronic signature is acceptable on all appointment forms. 
 
In the event you need a "Proof of Income" letter for your upcoming clerkship, please contact Beth Mason.

Special Instructions: Local Address
If you know the address that you will be using when you start employment, please indicate that address on all appointment forms. If you don't have your new address at the time you submit your appointment forms, please leave the address field blank and inform Beth Mason once you have your new address.
 
1. Application For Judicial Branch Federal Employment (A0‐78)
Complete this form accurately and entirely, as it is an important record of prior experience used to determine your qualifications and may be used in future consideration of opportunities for advancement. Under the Work Experience section, begin with your most recent position and work back in time. Make sure to include the month‐day‐year of employment. When indicating the number of hours worked per week, please be specific. If you obtained your J.D., please list the month-day-year in which you received your J.D. Also, make sure to indicate all legal experience including paid or unpaid work.

2. Employee Onboarding Form

Please complete the "Employee Onboarding Form" and return to Beth Mason at your earliest convenience. 

3. Employment Eligibility Verification (Form I-9)
In order to access the I-9 form, you must go to http://www.uscis.gov/i-9 Form I-9. Please let Beth Mason know if you have any issues accessing the form. Please complete Section 1 of the form. You are required to provide proper identification for verification prior to your start date. Email Beth Mason the following document(s) for verification of the I-9 form: one form of identification from List A or two forms of identification (one from List B and one from List C). Expired documentation will not be accepted. 

4.  Tax Forms
*IMPORTANT NOTICE* The address entered on the W-4 is the "address of record" used for mailing the W-2 at the end of the year. If an out of state address is listed on the W-4, withholdings for that state may apply.

Federal W-4: All employees must complete the W-4 Employee's Withholding Certificate form. 

State of Delaware taxes: Will be withheld from your pay regardless of state of residence. Delaware does not have a reciprocal agreement with Pennsylvania, New Jersey or Maryland. All employees must complete a "State of Delaware W-4" form. 
City of Wilmington Tax: Will be withheld from your pay regardless of state of residence. All employees must complete a "City of Wilmington Tax Form."

Residency Certification Form (PENNSYLVANIA RESIDENTS ONLY)
Per the Payroll Services Branch at the Administrative Office, Pennsylvania residents are required to complete a Residency Certification Form. Please fill in your mailing address, social security number, work address and sign/date the form. The Payroll Services Branch will complete the Resident PSD and Total Resident EIT Rate sections based on your municipality.

New Jersey residents ONLY: All residents of New Jersey are required to complete a NJ-W4 form.

Maryland residents ONLY: All residents of Maryland are required to complete an MW507 form. 
 
5. Direct Deposit Sign-Up Form (SF-1199A)
Direct Deposit is mandatory for all government employees. In section 1, enter your social security number in Item C (Claim or Payroll ID Number). Please make sure to enter your account type in Section D and account number in Section E. In Section 3, the Routing Transit Number is a 9-digit number. Be sure to type the last digit of your Routing Transit Number in the "Check Digit" field.  IT IS NOT NECESSARY TO PRESENT THIS FORM TO THE BANK FOR VERIFICATION IF YOU CAN ACCURATELY PROVIDE YOUR ACCOUNT AND ROUTING NUMBER.
  
6. BACKGROUND CHECK
As a condition of employment, background checks are mandatory for all newly appointed employees. Please note that the position for which you have applied is considered a sensitive position. If selected, you will be hired provisionally pending the outcome of the background check. The background check is an FBI Fingerprint Check of criminal history records. In preparation for your FBI Fingerprint Check at your onboarding appointment or on your first day of employment, please complete the Demographic Information and return to Beth_Mason@ded.uscourts.gov. 

7. DISTRICT COURT ONLY (do not submit the below forms if you are a Court of Appeals employee)

a. Consent to Criminal Records Check: In addition to your FBI background check, the District Court also requires all employees complete an NCIC background check prior to receiving the results of the FBI background check. Please complete the Consent to Criminal Records Check and return to Beth_Mason@ded.uscourts.gov. 

b. Information Technology Appropriate Use and Security Policy & Agreement. Please read the Information Technology Appropriate Use and Security Policy & Agreement  and return a signed copy of the agreement to: Elizabeth_Mason@ded.uscourts.gov

c. Emergency Information Form. Please complete the Emergency Information form and return to Beth_Mason@ded.uscourts.gov. This information will be used to set up an account for you on the Judiciary's Infoweb so you can be contacted in the event of an emergency.

9. PREFERRED NAME: If you have a preferred name you wish to have listed (Outlook, network log in), please advise of your preferred first name.

10. AO-78B Voluntary Race/Ethnicity, Gender, & Disability Identification
